Mr. David Richards, LCSW
David is a compassionate Licensed Clinical Social Worker with a wealth of experience supporting individuals with mental health challenges and ex-offenders. His approach focuses on identifying clients' strengths to help them tackle behaviors that hinder their well-being. With a gentle demeanor and strong communication skills, David builds meaningful connections to foster progress.
David has a strong background working with adult ex-felons with co-occurring disorders, and played a key role in developing the HMHC AB109 Re-Entry Program, collaborating with the LA County Department of Probation and DMH to ensure successful community reintegration. Currently, he serves as a Clinician at the Richard J Donovan Correctional Facility in the ML CCCMS Program in San Diego CA, where he works with some of the most complex cases. David’s goal is always to meet clients where they are and guide them toward a healthier, more fulfilling life.

Taylor Richards, Program Director
Taylor Richards serves as the Program Director for the ECM and Community Supports programs, where she provides leadership and oversight to the LCM team. She works closely with the Outreach Coordinator to ensure effective outreach to Molina Medi-cal members. Taylor focuses on developing and growing the program, including hiring and onboarding new Case Managers and Outreach team members. She brings strong leadership, a deep understanding of community-based care, and a passion for driving meaningful outcomes for the members served.
In her free time, Taylor enjoys spending time with her husband, children, and her cat.

Outreach Coordinator
As an Outreach Coordinator, David helps lead and coordinate outreach efforts to connect eligible members to the ECM program, ensuring they receive the services and support they need. David takes pride in building strong relationships with our members and supporting the success of the team.
Outside of work, David is a passionate sports fan who enjoys golfing and spending quality time with his fiancée.

Ashley Toscano, LCM Supervisor
Ashley Toscano serves as a Supervisor for the ECM team, guiding a group of LCMs to ensure they bring quality care to the members and foster great relationships with them. Drawing from her UCLA background and sociological lens, she inspires collaborative leadership centered on understanding social dynamics and uplifting client experiences. Throughout her time with the program, she has been instrumental in helping the team better manage client relationships and ensure everyone receives the help they need. In her free time, she enjoys trying new Cafe Shops and traveling to new areas with her family.

Janae Richardson,
LCM Supervisor
Janae Richardson attended Clark Atlanta University and has been serving as a Lead Care Manager since August 2024. With a passion for youth and community, Janae actively gives back through church volunteer work and is dedicated to making a positive impact in the lives of others. Driven by compassion and purpose, she loves the meaningful work of care management and the opportunity it brings to support and empower individuals and families.
