What is Enhanced Care Management (ECM)?
Enhanced Care Management is a Medi-Cal benefit that provides extra support to members with complex health and social needs. Each member is paired with a dedicated Lead Care Manager who helps coordinate medical care, manage medications, arrange appointments, and connect to community resources like housing, food, and transportation.
Who qualifies for ECM services?
ECM is available to Medi-Cal members who have serious health conditions or social needs that make it hard to manage their care. This includes populations such as people experiencing homelessness, those with frequent hospital visits, individuals with serious mental illness or substance use disorders, and children/youth with complex medical needs
How much does ECM cost?
ECM is a covered benefit under Medi-Cal. There is no cost to eligible members.
How do I enroll in ECM?
Most members are referred by their health plan (like Molina Medi-Cal) or their doctor. You may also reach out to us directly, and we can help check eligibility and connect you to services.
What can a Lead Care Manager help me with?
Your Lead Care Manager works with you and your providers to make sure your care is coordinated. This includes:
Helping schedule and prepare for doctor’s visits
Managing medications and treatment plans
Connecting you with housing, food, transportation, and other social services
Supporting you in reaching your personal health and wellness goals
Do I keep my current doctor if I enroll in ECM?
Yes. ECM does not replace your primary doctor or specialists. Instead, our team works alongside your providers to make sure all parts of your care are connected.
